I have read the protocols for requesting rooms at Knox during COVID-19 and agree to adhere to all the requirements.

Please complete and submit the following form. Your request will be reviewed by the Knox Office as soon as possible. We will contact you to confirm your request or to alert you to any conflicts. If you have additional questions regarding room requests contact Verna Hinman, Office Manager.

Please indicate requested room(s) and times.
It’s important to include set-up and tear-down time, (e.g. if your event starts at 2:00 PM and you need half an hour to set up, please select 1:30 PM as your start time).

Please include start and end times.
Please park in the lower level lot.
